ALL UNDERGRADUATE STUDENTS
Before beginning their first quarter, all undergraduate students must submit the following:
- 病史 Form
- Consent Form (signed by parent/guardian if under 18)
- Official documentation of 两剂MMR疫苗 (麻疹疫苗) 1岁以后给予 OR a positive antibody titer.Vaccine documentation should be signed or stamped by the vaccine provider 并且清楚地显示 学生的名字 和 出生日期.
A registration hold will result if these requirements are not met. Please allow 3-5 days for records processing before holds are removed.

登录安全 病人门户 with your SPU credentials (do not share your SPU credentials with anyone as this provides access to your health records). We strongly encourage students to do this for themselves even if parents help to provide immunization records.
- Select 表单标签.
- 下 行政 - please select 和 complete the 病史 Form Student 和 学生同意书. Parent must sign if under 18.
- 提交你的答案. 任何带有 红色* 是必需的. Allergies, medications, 和 significant medical history are particularly important.
#2: 免疫接种 Documentation for Measles
登录 病人门户 with your SPU credentials.
- Select 免疫接种 选项卡.
- If the required vaccine dates are not already entered, manually enter the MMR dates under 必需的免疫接种
- Select 上传 然后 免疫接种史 to 上传文件的副本 官方文档 (强制) from the student's health care provider, vaccine provider, or Washington State 免疫接种 Registry 卫生服务 staff will review this documentation 和 verify the appropriate vaccine dates.
#3: 上传 保险 Card
- Select 上传, 然后 保险 to upload your insurance information or insurance card photo if you have health insurance.
- This is not required, but highly recommended
特殊情况:
Vaccine exemption Requests: SPU免疫政策 allows students to request a 医疗豁免 (PDF)或 宗教豁免 (PDF) from the MMR vaccine requirement by completing 和 uploading the appropriate form with supporting documentation. 上传 the completed request form by going to 上传 和 selecting the waiver type requested from the drop down menu. Students must submit completed exemption request forms to 卫生服务 at least 2 weeks before registration opens for Fall quarter. Incomplete request forms will not be accepted. Students will receive a message through the patient portal confirming the status of their exemption request within 2 weeks.
国际学生: All International SPU Students (graduate 和 undergraduate) are required to meet the 健康 登记 requirements outlined above.
仅限在线学生: Students who are registering for only online classes 和 have no plans to be on campus for training, 会议, evaluations or for any other campus activities (including internships off campus) can request a temporary waiver from the immunization requirements. 的 网上豁免表格 is completed by the student, 和 reviewed 和 approved by the faculty or program advisor. This exemption is good for one quarter only 和 needs to be resubmitted each quarter that the student is registered for classes only online. Students who are in internships (not just online only classes) need to follow up with their faculty or program advisor to review the internship site requirements. This may require additional documentation.
